Description

In order to tryout and play for Forest Lake Academy Athletic teams, all forms in the Athletic Packet need to be filled out and signed with the appropriate signatures. The forms are:

Preparticipation Physical Evaluation (pdf). All 3 pages must be signed by doctor. Due before Tryouts. Athletes cannot tryout unless this form is turned in. The following forms are due upon making the team after tryouts
Consent and Release from Liability Certificate (pdf).
Athletics Consent.
Player Contract.
FLA Athletics Code of Conduct for Parents.

Athletic Fees are due shortly after the athlete makes the team.
