Medical declaration
I hereby certify that I am physically fit to participate in Masters Hockey, and related team participation; and that I have not been informed otherwise by a medical physician.
Indemnity and ackowledgement of riskI acknowledge that I am aware of all the risks inherent in Masters Hockey (training, trials and competition) including possible injury, disability or death, and agree to assume all those risks by signing this form.
I waive my rights to claim against any SOUTH AFRICAN MASTERS HOCKEY ASSOCIATION (SAMHA), SOUTHERN GAUTENG MASTERS HOCKEY ASSOCIATION (SGMHA) or the host venue, their officers, members, coaches, managers, players, servants and agents, and indemnify the same in respect of all claims arising from or connected with injury, disability, death or loss which I might sustain while, or as a result of, participating in the Masters Hockey and/or organised team practices for the tournament or related activity, whether caused by negligence or otherwise.
