Spartan Waiver and Consent

Waiver Summary Text:

LIABILITY and CONSENT WAIVER

In consideration of Spartan Soccer et al (hereinafter referred to as Spartan Soccer) permitting me to participate in its activities and sports, permitting me to the use of its equipment and permitting me the use of its facilities, I hereby agree as follows:

As an ADULT or ADULT Volunteer in the program/s:

TO WAIVE ANY AND ALL CLAIMS that I have or may in the future have against Spartan Soccer, and its directors, officers, employees, agents, representatives, assigns and successors.
TO RELEASE SPARTAN SOCCER, and its directors, officers, employees, agents, representatives, assigns and successors from any and all liability for any loss, damage, injury or expense that I may suffer or that my next of kin may suffer, as a result of my participation in activities and sports offered by SPARTAN SOCCER, due to any cause whatsoever INCLUDING NEGLIGENCE, BREACH OF CONTRACT, BREACH OF STATUTORY DUTY OF CARE, AND/OR BREACH OF THE OCCUPIER'S LIABILITY ACT, R.S.A. 2000, C. O-4, ON THE PART OF SPARTAN SOCCER and its directors, officers, employees, agents, representatives, assigns and successors.
TO HOLD HARMLESS AND INDEMNIFY SPARTAN SOCCER, and directors, officers, employees, agents, representatives, assigns and successors from any and all liability for any property damage or personal injury to any third party, resulting from my activities and my participation in the activities offered by or associated with SPARTAN SOCCER.
That this Agreement shall be effecting and binding upon any heirs, next of kin, executors, administrators and assigns in the event of my death.
Consent is also given for the use of and/or posting of still and action photos or video with/without player identification for the purpose of promoting soccer by Spartan Soccer and affiliated organizations and sponsors.
l have read and understood this Agreement prior to signing it. I am aware that by clicking the "I Accept" button below, I am waiving certain legal rights which I or any heirs, next of kin, executors, administrators and assigns may have against SPARTAN SOCCER and its directors, officers, employees, agents, representatives, assigns and successors.

As a PARENT AND/OR GUARDIAN of a MINOR or MINOR VOLUNTEER in the program/s:

RELEASE OF LIABILITY, WAIVER OF CLAIMS FOR SPARTAN SOCCER FACILITIES: I AM THE PARENT AND/OR GUARDIAN OF THE ABOVE REGISTERED MINOR(S) I AGREE THAT, IN CONSIDERATION of myself and the minor(s) being permitted to enter and use any one or more of any Spartan Soccer sanctioned events and premises (including all facilities and equipment located therein) for any activities including but not limited to soccer.
I AGREE that I, the undersigned, on behalf of myself and the minor(s), our heirs, successors and assignors, HEREBY REMISE, RELEASE, INDEMNIFY, DISCHARGE, AND FOREVER HOLD HARMLESS the Spartan Soccer Association(s)/league(s) organizing the game or event, their directors, employees, volunteers, coaches, instructors, agents and independent contractors and their heirs, successors and assignors from any claims whatsoever arising by reason of any disease, deterioration of health, illness or injury to any person, including death, or for damage to, or loss of any of my property resulting from or arising from use of the lands and premises, from being present on the lands and premises, from participation in any program, from the use of any facilities or equipment located on the lands and premises, from acceptance of the advice of, or from the negligence of the Associations and the association(s)/league(s) organizing the game or event, their directors, employees, volunteers, coaches, instructors, agents, independent contractors or any other persons using the lands and premises.
