Waiver of Liability and Player Acknowledgement

1. If you acceptec participate in the Southwest Flag Football League or any tournaments, I agree to release, indemnify, and hold harmless, Southwest Flag Football League, officials, referees, sponsors, and/or any of their employees, agents, officers, directors and representatives from any and all claims, causes of action, lawsuits and/or any type of judgement that may arise out of any injury to the above-named participant on this team during SWFFL or any related event or activity. I certify that I am covered by an approved medical insurance plan as required to play Adult Flag Football. I recognize and acknowledge that adverse weather conditions are an act of nature and agree to accept the decisions of the tournament officials as to playability and therefore, the outcom

2. The risk of injury from the activities involved in this program is significant, including the potential for permanent injury and death, and while particular skills, equipment, and personal discipline may reduce this risk, the risk of serious injury and/or death does exist.

3. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE

RELEASEES or others, and assume full responsibility for my participation.

4. If any player is found in possession of a weapon (e.g., knife, gun, heavy objects, baseball bat, axe, spear) on the field or sidelines during SWFFL events or league play, they will be suspended from league participation for one year. Additionally, any player who threatens the use of a weapon, whether witnessed by an SWFFL representative or supported by credible evidence, will also face a one-year suspension. A second offense of any kind will result in the player’s permanent removal from league play.

5. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual

significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the Tournament officials.

6. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE, INDEMNIFY, AND HOLD

HARMLESS Southwest Flag Football League, officials, referees, sponsors, or any other representatives their officers, officials, agents and/or employees, volunteers, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessors of premises used for the activity ("Releasees"), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property associated wit
