Parent ContractAs the parent of a CMS student-athlete, I recognize the commitment involved in my child's participation in the CMS athletic program. With this in mind:
* I will support my student-athlete in their academic responsibilities.
* I will support my student-athlete at competitions as often as possible.
* I will not schedule non-CMS activities during CMS practice and game times to permit my student-athlete full participation in all practices and games.* I permit my student-athlete full participation in all practices and games and give full permission for my child to ride CMS transportation to away games for the full school year.
* I will provide my student-athlete with all necessary personal clothing, protective gear, and equipment.
* I will support the decisions of the coaches and the Athletics Office.
* I will refrain from being a sideline coach/referee.
Hold Harmless and Indemnification Clause
In consideration of my child(ren) being permitted to participate in any athletic activities, programs, events, practices, or competitions organized, sponsored, or operated by the Carol Morgan School Athletics Department, I, the parent/guardian, hereby agree to the following:
Assumption of RiskI acknowledge and understand that participation in athletic activities involves inherent risks, including, but not limited to, physical injury, illness, permanent disability, or death. I voluntarily assume all such risks, whether known or unknown, and accept full responsibility for my child's or children's participation.
Release and Hold HarmlessI, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin, hereby release, waive, discharge, and agree to hold harmless and indemnify the Carol Morgan School, its athletic department, directors, officers, coaches, employees, volunteers, and agents, from and against any and all claims, actions, damages, liabilities, costs, or expenses (including attorney’s fees) arising out of or related to my child's or children's participation in any athletic activities, whether caused by negligence, omission, or otherwise, to the fullest extent permitted by law.
Medical AuthorizationIn the event of injury or illness, I authorize the Carol Morgan School Health Office to secure any necessary medical treatment for my child or children and agree to be financially responsible for any associated costs.
Compliance with RulesI agree to abide by all rules, regulations, and instructions of the Carol Morgan School Athletics Department and understand that failure to comply may result in suspension or termination of my child's or children's participation.
By checking the box, I acknowledge that I have read this Hold Harmless and Indemnification Clause, fully understand its terms, and voluntarily agree to be bound by it.
