I, ______________________________ (print your name) realize that I must adhere to the Constitution, Rules and Regulations of the West End Soccer League (WESL). I will especially ensure that following issues and matters are considered:
I will follow the "Code of Conduct" presented in the WESL Rules and Regulations regarding "Fair Play and Sportsmanship" along with other expectations. I am aware that violating the rules may result in my suspension and/or termination as a participant in the WESL. 
I must follow and support the regulations for use of the facilities/fields of the Pleasant Valley School District (PVSD) specific to 1)Use of the approved parking areas only. 2) No smoking on school grounds. 3) No pets on school grounds. I must also ensure that my relatives and friends that attend soccer activities of the WESL on PVSD grounds adhere to these rules. Violation of the PVSD regulations may result in my suspension and/or termination from participation in the WESL. 

I clearly understand and accept the purpose and goals of the West End Soccer League (WESL Constitution, Article II, Section 1) "The purpose of the West End Soccer League shall be to develop and direct the ideas of good sportsmanship, honesty, loyalty, courage and respect for authority. This will be accomplished through participation in youth soccer.
