If you haven't had the opportunity yet (or have not notified me that you have completed it), please take the time to complete one of the two REQUIRED Concussion Courses (CDC or NFHS); upon completion of the course, there is a short 3-4 question "test" and once that is done you will be prompted with the completion certificate. That is what I need sent to me (Rob Martin). I also need a signed Lindsay's Law Signature Form. It is OK to print it, sign it and either scan/email it to me or email a picture of it to me. The links to these are in the upper left hand corner of our home page under "Quick Links". If you have any questions, please let me know at rmartinhhs2015@gmail.com.
