Waiver Summary Text: I hereby authorize the staff of the 1018 Lions Basketball Program to act on behalf of my child, according to their best judgment, in any emergency requiring medical attention. I hereby waive and release the 1018 Lions Basketball Program staff from any and all liability of any injuries or illnesses incurred during training or resulting from attending the program.

I hereby grant 1018 Lions Basketball, its affiliates, and authorized media partners the irrevocable right to photograph, record, or otherwise capture my child's likeness, voice or image during all practices, games, and official events. These recordings and images may be used for promotional, public relations, and educational purposes including but not limited to team websites, social media, print materials and broadcasts.

I understand that all sales are final. No refunds will be provided.
