925 Sports Waiver and Release Form

Release of Liability

I, the undersigned, acknowledge and agree that participation in 925 Sports involves inherent risks of injury or property damage. I hereby release and discharge 925 Sports/Champion Christian Center, its directors, employees, volunteers, and affiliates from any and all claims, demands, or causes of action arising out of or connected with participation in the camp.

Physical Fitness Certification

I certify that my child is physically fit and has no medical or health conditions that would prevent participation in the activities of 925 Sports. 

Waiver of Rights

By signing this form, I acknowledge that I am voluntarily giving up the right to bring legal action against 925 Sports/Champion Christian Center or its staff for any injuries or damages sustained during participation. I understand the terms of this waiver and am signing it willingly.

Medical Consent

In the event of a medical emergency, I authorize 925 Sports staff to arrange for emergency medical care for me/my child. I understand that every effort will be made to contact the provided emergency contact first. I accept responsibility for any associated medical costs.

Photography Release

I give permission for 925 Sports/Champion Christian Center to use photographs or videos of me/my child for promotional purposes, including on social media, websites, and other materials.
