Description

Valid for one participant and provides access to The Ville Academy during open gym hours for a single day. Participants 17 years of age and under may use the gym for practice, shooting, and skill development during designated open gym times. Day passes are non-transferable and may not be shared between participants.

As the parent or legal guardian of the minor participant named above, I acknowledge that I have carefully read and understand the contents of this Agreement.

I voluntarily consent to the minor participant's involvement in activities conducted by The Ville Academy.

To the fullest extent permitted by law, I assume all risks associated with the minor participant's participation and agree to the terms and conditions of this Agreement on behalf of both myself and the minor participant.

By entering my name and submitting this form electronically, I certify that I am the parent or legal guardian of the minor participant and that my electronic signature has the same legal effect as a handwritten signature.

By signing and submitting this registration, I acknowledge that I have read, understood, and agree to all terms and conditions contained in this Agreement.
