Description

This membership allows one child (17 years old or younger) to visit The Ville Academy and participate in open gym activities, including practicing, shooting around, and using designated fitness equipment during any open gym hours throughout the month. Parent or guardian consent is required for all participants under 18 who wish to use the fitness equipment. Memberships automatically renew each month and will continue until canceled by the member.

As the parent or legal guardian of the minor participant named above, I acknowledge that I have carefully read and understand the contents of this Agreement.

I voluntarily consent to the minor participant's involvement in activities conducted by The Ville Academy.

To the fullest extent permitted by law, I assume all risks associated with the minor participant's participation and agree to the terms and conditions of this Agreement on behalf of both myself and the minor participant.

By entering my name and submitting this form electronically, I certify that I am the parent or legal guardian of the minor participant and that my electronic signature has the same legal effect as a handwritten signature.

By signing and submitting this registration, I acknowledge that I have read, understood, and agree to all terms and conditions contained in this Agreement.
