Description

🏀 Monthly Training with Coach Jason

This program includes two 1-hour training sessions per week with Coach Jason for four consecutive weeks (8 total training sessions). Participants will focus on skill development, shooting, ball handling, footwork, and overall basketball performance in a structured training environment.

Available training times are:

• Mondays & Wednesdays: 7:00 AM - 8:00 AM
• Mondays &Wednesdays: 8:00 AM - 9:00 AM
• Mondays & Wednesdays: 9:00 AM - 10:00 AM
• Mondays & Wednesdays: 10:00 AM - 11:00 AM
• Mondays & Wednesdays: 6:00 PM - 7:00 PM
• Mondays & Wednesdays: 7:00 PM - 8:00 PM
• Mondays & Wednesdays: 8:00 PM - 9:00 PM

After registration is completed, Coach Jason will contact you directly to confirm your preferred training time slot and finalize your schedule.

To the fullest extent permitted by law, I assume all risks associated with the minor participant's participation and agree to the terms and conditions of this Agreement on behalf of both myself and the minor participant.

By entering my name and submitting this form electronically, I certify that I am the parent or legal guardian of the minor participant and that my electronic signature has the same legal effect as a handwritten signature.

By signing and submitting this registration, I acknowledge that I have read, understood, and agree to all terms and conditions contained in this Agreement.
