All players must submit a completed Medical Waiver as part of registration each season. Click here to access the Medical Waiver. All players must provide a Medical Waiver either during registration or bring it to practice on the first day or they will not be able to participate.
USAU Membership
All players are required to have a 2025 USAU membership and complete the Release of Liability Waiver and the Infectious Disease Waiver on the USAU website. Your membership level and number must be included in your registration. Click here to access USAU
If you are new to Ultimate, you will need to create an account and purchase a membership and complete the Release of Liability Waiver and the Infectious Disease Waiver on the USAU website.
If you are returning but do not have a 2025 membership, you will need to login and renew your membership as well as complete a current Release of Liability Waiver and the Infectious Disease Waiver on the USAU website.
Scholarships
Any players interested in applying for a scholarship towards their registration and tournament fees, please email NVUltimateTreasurer@gmail.com for more information. Please note, requests must be submitted no later than one week prior to the first day of practice for consideration.
