2025 Fall Coaches Registration

Coaches/ Managers please complete the background forms for all of your coaching staff, 4 coaches and 1 team mom or team manager. Staff that does not have this requirement will not be allowed on the field. Coaches/ Managers must also submit a complete player roster and must present the players birth certificates during the coaches meeting prior to the start of the season. Players that have not had their birth certificate verified will not be eligible to play until they have been verified by a league official. All teams are required to have Game Changer.
